Your next step after attending your Initial Consultation appointment, is to follow your Roadmap. Every patient’s Roadmap may be slightly different in terms of the requirements for office visits, clearances, tests and timing. Therefore, we will provide you with details and your Patient Advocate will be your partner to be sure you are moving along your journey.
Please let us know if you need your Patient Advocate. This is the person who can check on tests, receive paperwork, and reschedule any appointments for you.

Please understand, our staff is here for you and all of our patients. Arrive at your appointment 15 minutes early to be sure you are on time. In the event you must cancel an office visit, please do so 24 hours in advance to avoid a $25 cancellation fee.
Our Program
Our program involves lifestyle change. This is led by our team approach. You will meet and see our Exercise Physiologist, Patient Advocate on a regular basis to help change your lifestyle.
